Consent For In Office Labs

I understand that I have chosen to have Regenerative Wellness Center draw my blood to do labs in the office through Ulta labs. I understand that I will owe an additional fee for labs ordered by Dr. Warren, that was not included in the consultation fee, at my following appointment to review them. Labs are proprietary until paid for. At that time, they can be released to patient if asked.
